Covid 19 – Please read prior to your appointment

We would like to make you aware of a few changes at our practice due to the latest Government COVID-19 Regulations.

Prior to your appointment, please ask yourself

  1. Do you have a fever, or have you felt hot or feverish within the last  14-21 days?
  2. Do you have a new or continuous cough?
  3. Have you experienced recent loss of taste or smell?
  4. Do you have any other flu symptoms or illness?
  5. Does anyone in your household have any symptoms of COVID or had a positive test in the last 10 days?
  6. Have you been contacted by Test and Trace in the last 10 days?
  7. Have you been in contact with anyone who has had COVID-19 confirmed within last 10 days?

If you have answered ‘yes’ to any of these questions, please contact the practice prior to your appointment as we may need to postpone seeing you.

If you answered ‘no’ to all questions, then we look forward to seeing you at your appointment.

  • You must wear a face mask (unless medically excluded). Whilst in the practice please ensure your face mask is worn at all times except when in the surgery chair.
  • If possible, complete your medical forms online via the email sent prior to your appointment. If you do not have access to emails, please advise us and a paper copy will be ready for completion upon your arrival
  • Hand sanitiser will be provided and must be used upon entry and departure
  • Our toilet is available if required – Please inform a staff member if you need to use, in order that we can sanitise after your use

COVID-19 related questions will be asked upon your arrival. We would just ask you to please be kind, patient and respectful as we try to keep everyone safe.
